Timothy Noakes
The Trial That Changed Everything
Timothy Noakes is an exercise physiologist, physician, author, and one of the most prominent figures in the modern history of low-carbohydrate nutrition. For much of his career, he was closely associated with endurance physiology and the conventional carbohydrate-loading model, particularly through his influential textbook Lore of Running.
His later change of position was therefore especially significant. Noakes did not begin as an outsider to the prevailing nutritional paradigm. He had helped teach it. His own metabolic deterioration, including weight gain, fatigue, and pre-diabetic markers, forced him to re-examine assumptions he had previously accepted.
That reconsideration led him towards carbohydrate restriction, fat adaptation, and metabolic flexibility. It also placed him in direct conflict with established dietary institutions.
In 2014, the Health Professions Council of South Africa initiated proceedings against Noakes following his public response to a breastfeeding mother on social media. He was accused of unprofessional conduct for advising that her child's health would be better served by a low-carbohydrate, high-fat diet.
The case extended across several years and placed his professional standing at risk. In 2017, the disciplinary committee cleared him, finding that the complaint had failed to establish that his advice was not evidence-based. The decision was subsequently appealed.
In June 2018, a second independent committee dismissed the appeal unanimously. Noakes had successfully defended not only his own conduct, but the broader principle that evidence-based dietary advice does not become professionally illegitimate merely because it departs from official orthodoxy.
His contribution to The Power of the Lion therefore extends beyond physiology alone. Noakes represents the institutional and legal struggle surrounding metabolic medicine: the point at which a scientific disagreement became a test of whether established authorities could define the boundaries of acceptable evidence.
His later editorial work on Ketogenic: The Science of Therapeutic Carbohydrate Restriction in Human Health helped consolidate the modern scientific case for therapeutic carbohydrate restriction within a comprehensive clinical and academic framework.
